Join us at HumanKind Foster Care and become a part of the team that values compassion, dedication, excellence, and a work culture you want to be a part of everyday Success In This Role Looks Like: Complete and maintain thorough documentation including but not limited to: Monthly Notes and Reports, 14 Day Plans, 30 Day Plans, 45 Day Plans, Social Histories, Quarterly Reports, and Annual Reports.Assess the needs of foster children, and implement tangible objectives, strategies and services to address all identified needs. Completion of home and field visits multiple times a month for each child. Attend or coordinate meetings on behalf of foster children and foster families including; FAPT, Court, FPM meetings, Treatment Teams, and VEMAT meetings. Supervise visitations between foster children and their birth families, provide transportation for identified services, and make service referrals to various agencies. Participation in ongoing trainings required by LCPA standards, HumanKind, and supervisor. Participation in all audits, to include internal and external conduct by LCPA Licensing Inspector. Use Placement/Matching protocol in the event assistant is needed for placement by the Program Manager. Comply with LCPA standards, HumanKind and HK TFC policies and procedures, and remain up to date on any changes.
